EQUASS Documents

Applications for EQUASS certification are submitted in writing. Forms and guidelines help with applicants prepare the complete set of documents necessary to conduct an audit. 

For EQUASS certification in 2026, applicants are invited to use the current set of templates and guidance notes referring to EQUASS 2018. Later in the year, the EQUASS National Representatives and EQUASS Office will present a revised EQUASS framework and documents, and transition the application process to a dedicated online portal. The new templates, documents and tools shall remain valid for six to eight years.
